Description: Summary of Responsibilities Provide quality services to support individuals in maintaining supported living in the community Specific Responsibilities – Essential 1.Communicate with designated staff regarding individual and program issues. Attend team conferences, when appropriate, providing observational information for evaluation and treatment planning purposes. Promote teamwork, problem identification and problem solving. Model and promote positive morale. 2.With the treatment team, implement treatment techniques to improve or maintain individual functioning in accordance with established treatment goals. 3.Promote community independence with consumers using notebooks, cues, and individual supportive assistance and structuring tasks. 4.If full-time, carry on-call phone as assigned and necessary. 5.Maintain a current on-call list and keep supervisor and CLP staff updated regarding changes as necessary. 6.Monitor individual progress and maintain accurate records of treatment through data collection and treatment reports as assigned. Complete timely documentation of client records such as incident and team update reports. 7.Develop appropriate and effective interpersonal style with all consumers. 8.Utilize adaptive equipment as appropriate to increase the consumer's functional independence level. 9.Participate in program development under the direction of the coordinators of the Community Living Program. 10.Enforce and follow program rules with assistance from the Interdisciplinary Team. 11.Monitor consumers' adaptive equipment and recommend repair, replacement, or new purchases. 12.Transport consumers if needed to medical/dental/equipment appointments. 13.Market the Community Living Program by keeping quality control and customer service at high levels. 14.Work with case manager/social worker to facilitate appropriate lines of communication between consumers, families, staff and management. 15.Other duties as assigned.
